Performance continued to vary widely across regions. While general volume trends were down, two regions stood out from a year-over-year perspective: the Great Plains, and the South. The Great Plains performed markedly better across all volume metrics, while the South tended to lag behind other regions. These variances are likely driven by state-specific payment, economic, and demographic trends, which may be exaggerated should the economy soften.

National Volume Observations
December saw positive volume performance relative to the prior month across Discharges, Adjusted Patient Days, Adjusted Discharges, and ED Visits, but volumes lagged behind budget expectations. Operating Room Minutes and Length of Stay were down month-over-month and relative to budget, indicating a likely drop in patient acuity for the period. Weaknesses in the year-over-year figures ran counter to the month’s profitability outcomes, indicating strong performance management and/or payer mix. Alternatively, it may highlight the potential for negative financial impacts in the coming months, as finances catch up to volumes. At a more granular level, the range of performance varied significantly—indicating that stronger organizations may continue to build on their strengths, while weaker organizations continue to fall further behind.

Hospitals of all sizes underperformed in discharges and ED visits relative to budget and prior year in December, indicating that individual facility size does not guarantee volume performance. The one outlier is Operating Room Minutes, where the largest hospitals exceeded budget expectations.
